Available to you free of charge, complete the online form to request sample shipping materials (e.g., swabs, vials) and animal crates to fulfill for your health surveillance testing needs. 

Complete the submission form (fillable PDFs are available below) and place a hardcopy in the box with your sample shipment.

Sample specifications and shipping instructions are described on the last page of each form. Prior to shipping live animals, review the Animal Safety Checklist and take the necessary steps to ship them properly.
